Our physicians, physician assistants and nurse practitioners care for the whole family — newborns, teens, parents and grandparents. We take the time to listen, get to know your history, and build a plan that fits your life.

Several of our family medicine physicians also provide obstetric care, so you can stay with a provider you trust from your first prenatal visit through delivery and beyond.

Telehealth visits

For many follow-ups and simple concerns, you can see your SRMC provider from home on a smartphone, tablet or computer. Telehealth saves you the drive while keeping you connected to the same team that knows you.

Chronic Care Management

If you live with two or more ongoing conditions, our Chronic Care Management program adds support between appointments. A team of nurses, social workers, case managers and your provider coordinate your care, and nurses check in by phone to catch new symptoms early.

Common qualifying conditions

  • Alzheimer's disease and dementia
  • Arthritis
  • Cancer
  • COPD
  • Diabetes
  • Heart failure
  • Osteoporosis

Transitional Care Management

Coming home after a hospital stay can be overwhelming. For eligible Medicare patients, our care management team reaches out within two business days of discharge and supports you through the following 30 days — reviewing medications, arranging follow-up and answering questions.

Anticoagulation management

With a referral from an SRMC primary care provider, our pharmacists can manage blood-thinner therapy in a collaborative practice with your provider — a service usually found only in large university hospitals.

Medicare Annual Wellness Visits

A yearly preventive visit to review your health, update screenings and plan ahead. It's a covered benefit for most Medicare patients and a great time to talk through goals with your provider.
