Fastest option
Request through My Health Connection

Log in to the UCHealth app and tap the menu (three bars) at the top right.

Open “Tools,” then choose “Share my record.”

Pick who the copy is for — yourself, or a family member, friend or caretaker.

Choose “Request a formal copy” (or “Download or send a snapshot”).
- 05
Answer the prompts and tap “Continue.” We'll message you when it's ready.
